Checklist to File a Re-Examination/Conversion Application (repeater, postponer, or converter)

This checklist will assist you in completing the Re-Examination/Conversion Application. Click the links below to access information or print forms. A ticket of admission will be issued upon receipt of a properly completed, notarized Re-Examination/Conversion Application with all required supplemental documents and fee(s) if postmarked on or before the timely filing deadline. Re-Examination/Conversion Applications are accepted after the deadline, but late filing fees (escalating until the application and all supplemental documents are received) will be required.

STEP 1 - PRINT AND USE CHECKLIST

____1.Print this Checklist
____2.Check each item when completed to assure that your Examination Application includes all necessary supplemental documents and information.

STEP 2 - GATHER AND PREPARE REQUIRED MATERIALS

Completion of the online Re-Examination/Conversion Application will take less than 15 minutes. It is suggested that you gather and have the information listed below available when you begin.
____1.Applicant file number.
____2.Law school name and date of graduation (month and year).
____3.If admitted in another jurisdiction, the date (month and year) first admitted.
____4.Fingerprints (unless previously submitted to IBT as a Florida Bar applicant, registrant, or CLI registrant)
All applicants for admission to The Florida Bar must have their fingerprints scanned for electronic submission to the Florida Department of Law Enforcement and to the FBI by Integrated Biometric Technology (IBT). Instructions for this step are in the answer to How do I submit the required fingerprints? in our FAQ. Confirmation of your compliance with this step must be received by the board before a ticket of admission into the General Bar Examination can be issued to you. If your fingerprints have already been taken by IBT, you are not required to be re-fingerprinted.

STEP 3 - COMPLETE THE APPLICATION ONLINE

When you have gathered the information above, you are ready to complete the Re-Examination/Conversion Application. A high speed Internet connection (LAN, DSL, cable modem, etc.,) as opposed to dial-up is recommended. Due to reports of problems, we also recommend that you do not try to complete the Re-Examination/Conversion Application with a Mac.
____1.Examination Application
Click the link above to access the on-line application in a new browser window. Answer the questions in order as some questions are based upon your responses to earlier questions.

____3.Print application
Print 2 copies of your Re-Examination/Conversion Application. Mail 1 to the Board and keep 1 copy for your records. Once you close your browser, the Re-Examination/Conversion Application will no longer be available to print. If you fail to print the application, you will have to re-enter all data from the beginning.
____4.Complete the handwriting sample on the last page of the application.
____5.Have the Re-Examination/Conversion Application notarized. The notary should require identification and place you under oath prior to requiring you to sign the document.
____6.Confirm that the notarization is properly executed (including that the notary properly indicates how you were identified, signs, dates and affixes his or her stamp). Incorrect notarization will cause your application to be returned and may result in your being charged an examination late filing fee or for your late filing fee to be higher.
